The consent given when submitting an enquiry through the site.

CONSENT TO THE PROCESSING OF PERSONAL DATA

MEDITEXT CONSULTING LLC

The contact form on the website operates not on consent but on the legitimate interest of the Controller (Article 11(1)(6) of Law No. 2297-VI): a person approaches us as a representative of a facility, on that facility's business, and provides business contact details in order to obtain a business reply. There is therefore no mandatory consent checkbox under the form; instead, the notice required by Article 12 of the Law is given.

Consent remains the basis in two cases: marketing communications, and data received outside the form (sections B and C).

A. NOTICE UNDER THE CONTACT FORM

The notice required by Article 12 of Law No. 2297-VI, shown beneath the submit button:

By sending this enquiry you provide business contact details to MEDITEXT CONSULTING LLC (EDRPOU 45551955) so that your enquiry can be handled. The basis is the legitimate interest of the Controller; you may object to the processing. Retention and your rights are set out in the [Privacy Policy](https://www.meditext.com.ua/legal/privacy).

A separate checkbox, for marketing only, not ticked by default:

The marketing consent checkbox is not ticked by default — consent is an affirmative act by the User. The fact, date and time it is given are recorded in the system.

B. FULL TEXT OF CONSENT

This text applies where consent is itself the basis of processing: marketing communications, and data provided by email, by telephone or otherwise outside the website contact form.

By providing my personal data I, the data subject, freely give consent to the Controller — MEDITEXT CONSULTING Limited Liability Company (EDRPOU 45551955, address: 53A Peremohy Ave., office 3-4, Kharkiv, 61201, Ukraine) — to process my personal data on the following terms:

  1. Data concerned: surname, first name, patronymic, organisation name and position, telephone number, email address, and any other data I provide voluntarily in my enquiry.
  1. Purpose of processing: handling enquiries and requests, providing consultations and commercial proposals, concluding and performing contracts for consulting services and, where separate consent is given, sending marketing communications.
  1. Processing operations: collection, registration, accumulation, storage, adaptation, amendment (updating, modification), use, anonymisation and destruction of personal data in accordance with Law No. 2297-VI.
  1. Disclosure and place of storage: I am informed that my data is accessible only to employees of the Controller and to processors acting on its instructions. The data is stored on a server rented from the cloud infrastructure provider DigitalOcean, LLC in Frankfurt am Main, Germany — a member state of the European Economic Area, recognised as providing an adequate level of personal data protection (Article 29(2) of Law No. 2297-VI). Where there are grounds to do so, data may be disclosed to project partners in the manner set out in the [Privacy Policy](https://www.meditext.com.ua/legal/privacy).
  1. Duration: this consent is effective for as long as is necessary to achieve the purpose of processing and may be withdrawn by me at any time by writing to admin@meditext.com.ua.
  1. Rights: I am informed of my rights under Article 8 of Law No. 2297-VI (access, correction, deletion of data, withdrawal of consent, appeal) and of the inclusion of my data in the Controller's personal data filing system.

The date and time consent is given are recorded automatically by the system at the moment it is given.

C. NOTICE UNDER ARTICLE 12 OF LAW No. 2297-VI

Article 12 requires that the data subject be informed of the controller, of the composition and purpose of the data collected, and of the persons to whom the data is disclosed — irrespective of whether consent is the basis. Notice for channels outside the website:
